Longitudinal clinical intelligence for radiology

Every prior, every finding, one cited view.

RAIN is being built to assemble a patient's prior imaging reports, link each finding across exams, and show how it has changed over time. It is designed to sit beside the current study, with every statement linked to the report sentence it came from.

EXAM: MRI BRAIN WITH AND WITHOUT CONTRAST
CLINICAL HISTORY: Follow-up of treated brain metastases.
COMPARISON: [not yet inserted]
FINDINGS: [radiologist dictates current findings]

This paragraph summarizes prior reports only. The radiologist adds the current exam's findings, reviews and edits every statement, and decides what goes into the signed report.

Workflow

A follow-up read, with the history already in place.

How RAIN is designed to work during a neuroradiology follow-up read.

01

The follow-up study opens

A brain MRI comes up on the worklist. The patient has five prior exams across two years, and the comparison field lists only the most recent one.

02

The priors are already assembled

RAIN is designed to retrieve the prior reports ahead of time and order them by date and modality, so there is nothing to search for and no separate window to open.

03

The timeline shows each finding's course

Each tracked finding sits on one row with its measurements across exams and its current state, so the findings that need attention today stand out.

04

Every finding links to its source

Select a finding to see the exact sentence from the prior report, with its date and section. If the summary and the source disagree, the source is one click away.

05

A cited comparison goes into the draft

RAIN proposes a comparison paragraph with a citation for each statement. The radiologist inserts it, edits it and adds today's findings. Nothing enters the report until it is signed.

Platform

Six modules, one longitudinal record.

RAIN runs as a pipeline: Retrieve, Connect, Reason and Surface, with Track planned next. Each module has one job in that sequence.

RetrieveIn development

Prior Assembly

Gathers the relevant prior reports and study records before the current study is read.

  • Collects prior reports and study metadata: date, modality, body region, protocol
  • Filters priors by relevance to the current exam
  • Orders exams on one timeline, with gaps and modality changes shown
ConnectIn development

Finding Graph

Identifies individual findings in each report and links the same finding across exams.

  • Extracts findings with location, size and descriptors
  • Designed to link the same lesion across reports despite changes in wording or modality
  • Designed to keep lesions separate, so a new lesion is not merged into an old one
ReasonIn development

Temporal Engine

Assigns each linked finding a state over time.

  • Classifies findings as new, growing, treated, stable or resolved
  • Uses treatment events in reports, such as radiosurgery or resection
  • Leaves findings unclassified when the state is uncertain
SurfaceIn development

Evidence View

Shows the source report sentence behind every statement RAIN makes.

  • Links each measurement and state to a report, section and verbatim sentence
  • Opens the full source report with the sentence highlighted
  • Labels any statement without a direct source as unsourced
SurfaceIn development

Comparison Draft

Proposes a cited comparison paragraph for the radiologist to review and edit.

  • Writes a short paragraph with citations mapped to specific priors
  • Inserts into the report draft only when the radiologist chooses
  • Keeps citations visible during editing for verification
TrackPlanned

Follow-up Tracker

Tracks findings that prior reports recommended for follow-up.

  • Records recommendations such as "MRI in 3 months"
  • Shows whether the recommended follow-up took place
  • Designed to support section-level review of open follow-ups
Design principles

Built for the reading room.

No extra steps to start

The summary is intended to be ready when the study opens, with no separate search and no uploads.

Sits beside PACS

RAIN is designed to run in a panel beside the viewer and the reporting system. The images stay at the center of the read.

Radiologist in the loop

RAIN proposes and the radiologist decides. Nothing is written to the report without an explicit action, and every sentence can be edited.

Every statement cited

Each measurement, state and sentence links to the prior report text that supports it. Anything RAIN cannot source is designed to be labeled as unsourced.

Integration and deployment

Designed to fit the systems you already run.

These are design goals. Integration and deployment details will be confirmed with each institution's clinical, informatics and security teams before any deployment.

PACS and RIS integration

Designed to retrieve prior study records through DICOM query and worklist context, and to open beside the current study without a separate login.

Report retrieval

Designed to read prior report text through HL7 v2 messaging or FHIR resources such as DiagnosticReport and ImagingStudy, depending on what the institution supports.

Reporting system handoff

Designed to insert the approved comparison paragraph into the draft at the radiologist's request, using the reporting system's supported integration points.

Deployment and PHI

Planned for deployment inside the health system's infrastructure or a private tenant it controls, with minimum-necessary data, encryption in transit and at rest, and role-based access.

Audit logging

Designed to log each summary generated, each source viewed and each paragraph inserted or edited, with user and time, to support quality review and validation studies.
